Job Summary
The Registered Nurse (RN) provides skilled nursing care in a home health and community-based setting. This role includes patient assessment, care plan implementation, health maintenance, disease prevention, and case management. The RN collaborates with interdisciplinary teams, supervises staff as needed, and ensures compliance with physician orders, regulatory standards, and agency policies.
Key Responsibilities
Assess patient health status and develop, implement, and evaluate individualized care plans
Provide nursing care for well, chronic, and acutely ill individuals in home or small facility settings
Coordinate care with physicians, therapists, and other healthcare professionals
Provide case management and ensure continuity of care
Monitor patient condition changes and communicate updates to physicians
Transcribe and ensure implementation of physician orders
Manage medication processes, including ordering, monitoring, storage, error management, and disposal
Ensure medication policies are current, compliant, and reviewed annually
Complete required daily, monthly, quarterly, and annual nursing documentation
Maintain accurate and timely nursing notes reflecting patient health status
Ensure proper infection control practices and safe maintenance of equipment and supplies
Supervise and direct Licensed Practical/Vocational Nurses as needed
Schedule and coordinate health-related appointments and follow-up care
Communicate with inpatient nursing staff and discharge coordinators during hospitalizations
Provide new employee orientation and annual competency training
Train unlicensed staff on person-specific health, safety, and disease prevention needs
Requirements & Qualifications
Active Registered Nurse (RN) license
Compact license accepted
Must have home health experience
Experience with OASIS documentation required
Familiarity with HCHB (Homecare Homebase) preferred
Wound care competency preferred (wound class available if needed)
BLS certification required
T-Step TB test required
Preferred Skills
Strong assessment and care coordination skills
Excellent communication and documentation abilities
Ability to work independently in community settings
Leadership and staff supervision experience
