Qualifications:

• Valid and active LPN license
• Minimum of one year of recent nursing experience (long-term care, acute care, or similar settings preferred)
• Current Basic Life Support (BLS) certification
• Strong assessment, communication, and organizational abilities
• Ability to work collaboratively in team-based settings
• Up-to-date immunizations and health screenings as required by facility policy

Primary Responsibilities:

• Administer medications and treatments as prescribed, observing patient responses
• Collect and document patient vital signs and observations in electronic medical records
• Assist Registered Nurses and physicians with assessments and procedures
• Deliver exceptional patient-centered care while maintaining safety protocols
• Support patient mobility, hygiene, and comfort needs
• Effectively communicate with patients, families, and interdisciplinary teams to facilitate care planning
