Registered Nurse (RN) | Case Manager

Salary: $85,000 plus $5,000 sign‑on bonus. Productivity bonuses: $100 per additional regular visit, $150 per SOC. Employment term: 1 year required to receive full sign‑on bonus.

Schedule:

Monday – Friday, 8 a.m. – 5 p.m.

Location:

Redding, CA and surrounding areas

Must have home health or hospice experience.

• Annual salary of $85k plus productivity bonuses
• 1:1 nurse‑to‑patient ratios
• Creates own schedule

Provide in‑home direct patient care according to the provider’s prescribed plan of care and nursing scope of practice, monitoring progress and recovery.

• Assess and document patient conditions, reporting changes as needed (vitals, lab values, glucose, etc.).
• Administer medication, insulin, IV fluids, and document thoroughly.
• Inspect wounds, change dressings, and assist with personal hygiene.
• Assess for injuries, infections, bedsores and deliver appropriate treatments.
• Collaborate with providers, aides, and allied health professionals to ensure exceptional outcomes.
• Participate in ongoing interdisciplinary assessment of the patient.
• Participate in Quality Assessment and Performance Improvement (QAPI) program.
• Supervise paraprofessional and licensed professional team members.

• Graduate of an accredited nursing school and a current California RN license.
• Two years of current nursing experience preferred.
• One year management experience preferred.
• Must have home health or hospice experience.
• Community health and broad medical‑surgical experience preferred, not required.
• Ability to pass a criminal background check, physical, TB and drug screen.
• Valid driver’s license and reliable transportation.
