Licensed Practical Nurse

Corning Center is seeking a compassionate Licensed Practical Nurse to provide direct resident care in our skilled nursing and rehab facility. The LPN will administer medications and treatments, monitor vital signs, document care accurately, and support residents with daily living needs. You will collaborate closely with RNs, physicians, therapists, and CNAs to follow care plans and ensure resident safety, comfort, and dignity.

The role involves communicating with families, reporting changes in condition, and maintaining infection control and regulatory compliance in a resident-focused, team-based environment.

Responsibilities

• Provide direct nursing care to residents in a skilled nursing and rehab setting
• Administer medications and treatments per provider orders and facility protocols
• Monitor vital signs, resident condition, and provide timely reporting of changes
• Document care, interventions, and resident responses accurately in medical records
• Assist residents with activities of daily living while promoting dignity and independence
• Collaborate with RNs, physicians, therapists, and CNAs to implement care plans
• Educate residents and families on care, treatment, and health maintenance as directed
• Maintain infection control, safety standards, and regulatory compliance
• Participate in care conferences, shift handoffs, and interdisciplinary team meetings
• Support a compassionate, resident-focused, and team-oriented care environment
