This full-time position provides strategic direction, implementation, and management ofCalAIMinitiatives, including Enhanced Care Management (ECM), Community Supports (CS), and behavioral health integration. This role ensures operational compliance with DHCS guidance, cultivates cross-sector partnerships, and advances equity-centered service innovation. Reporting directly to the Executive Director, this position is well-suited for candidates with comprehensive knowledge of MCPs, a commitment to health equity, and demonstrated ability to lead complex, multi-stakeholder initiatives.
- Ability to design, implement, and evaluate
CalAIMinitiatives across ECM, Community Supports, and behavioral health integration - Deep understanding of DHCS guidance, MCP regulations, and CalAIM policy updates
- Skilled in building partnerships across health plans, public health, housing, behavioral health, and justice systems
- Capacity to monitor service delivery, ensure fidelity to CalAIM standards, and manage audits or compliance reviews
- Commitment to culturally responsive care, health equity, and reducing disparities across populations of focus
- Ability to lead multidisciplinary teams, support staff training, and promote trauma-informed, person-centered care
- Proficiency in using performance metrics, quality improvement tools, and reporting systems to guide strategy
- Translate complex policy into actionable program design; advocate for system-level improvements
- Oversee funding streams, allocate resources effectively, and align operations with fiscal sustainability
- Lead organizational transformation, navigate resistance, and foster innovation in service delivery models
- Lead strategic planning, design, and execution ofCalAIMinitiatives across all service lines, including Enhanced Care Management (ECM), Community Supports, and behavioral health integration
- Develop and implement operational workflows, care pathways, and quality assurance protocols that ensure fidelity to DHCS requirements and MCP contractual obligations
- Build and maintain collaborative partnerships with Managed Care Plans (MCPs), county behavioral health departments, public health agencies, housing providers, and criminal justice partners
- Serve as primary liaison with MCPs for contract negotiations, service authorization processes, and performance improvement initiatives
- Collaborate with our affiliated organizations to align CalAIM strategies, share best practices, and coordinate cross-organizational service delivery
- Ensure organizational compliance with DHCS regulations, MCP contract requirements, and Joint Commission standards; prepare for regulatory audits and site reviews
- Directly supervise a team of Lead Care Managers (LCMs) and Community Health Workers (CHWs), and coordinate with the Clinical Consultant to ensure integrated, high-quality care delivery
- Develop and deliver staff training on CalAIM populations of focus, trauma-informed care, and culturally responsive practice
- Establish and monitor key performance indicators (KPIs) for program services, including engagement rates, service utilization, and health outcomes
- Utilize data analytics to identify trends, address service gaps, and inform continuous quality improvement initiatives
- Champion equity-centered approaches that address social determinants of health and reduce disparities in access and outcomes for priority populations
- Translate evolving DHCS guidance and regulatory updates into actionable policies, procedures, and training materials

- Master’s degree in Public Health, Social Work, Health Administration, or related field preferred or equivalent experience
- Active clinical licensure (LCSW, RN, LMFT, or similar) preferred but not required
