Registered Nurse

Bella Terra Home Health and Hospice is seeking an experienced, compassionate Registered Nurse (RN) to join our hospice team. The RN plans, organizes, and directs home care services and is experienced in nursing with an emphasis on community health education.

Pay Range: $50.00 - $65.00/hr

Develop and initiate the plan of care and assume responsibility for the ongoing interdisciplinary assessment and development of the individualized plan of care in partnership with the patient, representative, and caregiver(s). Initiate proper preventative and rehabilitative nursing procedures and provide services that are ordered by the physician as indicated in the plan of care. Provide patient and caregiver education. Identify discharge planning needs as part of the care plan development and implement prior to discharge.

Ensure arrangements for equipment and other necessary items and services are available. Supervise Licensed Practical Nurses (LPNs)/Licensed Vocational Nurses (LVNs) and/or Home Health Aids (HHAs).

• Develop and initiate plan of care.

• Provide patient and caregiver education.
• Identify discharge planning needs and implement prior to discharge.
• Ensure arrangements for equipment and other necessary items and services.
• Supervise LPNs, LVNs, and Home Health Aids.

• Active CA RN license.
• Minimum of one (1) year of direct patient care experience; preferable in hospice setting.
• Current CPR certification.
• Licensed driver with an insured automobile in good working order.
