** What You Will Do*
* Supervisory responsibility for the acute care hospital-based Transition Planning team. This position supervises, provides oversight and coordination of the day-to-day operations of Integrated Care Management for Sharp Coronado Hospital.  Coordinates the daily productivity, longitudinal patient care with a focus on seamless and coordinated transition planning; ensure quality outcomes and optimal utilization of available resources.  Supervises staff to ensure all regulatory requirements are met.

(IMM, Non-Coverage, Patient Choice, etc.)  Participates as a member of the ICM management team. Maintains an open mind with a passion for departmental growth and program enhancement.

** Required Qualifications*
* + Bachelor's Degree In a health care related field Or

+ Bachelor's Degree in Nursing

+ 5 Years Acute care or clinical experience in area of specialty

+ 2 Years Clinical experience in Utilization Review, Discharge Planning and/or Case Management in a hospital and/or Managed Care setting.

+ California Registered Nurse (RN) - CA Board of Registered Nursing
