We are seeking a Pharmacist to join our team of passionate patient caregivers!

Shift: Alternating weekdays and/or weekends. Most evenings 5PM to 10 PM and some days 10AM to 6PM

Required Skills:

• Doctor of Pharmacy from an ACPE accredited college of pharmacy
• At least one year of hospital pharmacy experience
• Supervisory experience preferred.

• Ensures safe transitions of care by completing comprehensive medication reconciliation and final verification of admission and discharge orders for all patients.
• Evaluate and verify Well Sky physician s orders for therapeutic appropriateness, proper dosing, optimal dosage form, drug interactions, allergic reactions, route and rate of administration and therapeutic redundancy.
• Clarifies physicians  orders, when necessary, through direct and timely contact with prescriber or designated practitioner.
• Demonstrates knowledge with antimicrobial stewardship, which includes therapy optimization, de-escalation, and selection of appropriate, evidence-based antimicrobial regimens.
• Assists with monitoring and appropriate prescribing of anticoagulation therapy.
• Provides patient-specific pharmacotherapy recommendations and drug information to the medical and nursing staff.
• Supervises pharmacy technicians in activities related to medication dispensing, ordering and inventory management.
• Thorough documentation of all clinical interventions.
• Exemplifies the Ernest Health mission, vision and values and acts in accordance with Ernest Health policies and procedures.
• Uphold departmental and institutional policy as appropriate.
