SummaryThe Licensed Practical Nurse in Home Health is responsible for providing quality patient care and performing technical skilled care in the patient's home.
ResponsibilitiesClinical Competence
Provides high-quality clinical services within the scope of practice, as defined by state laws, in accordance with the plan of care, and in coordination with other healthcare team members, following all applicable infection control procedures. Does not alter the plan of care.Continuously observes and reports the patient’s nursing needs to the supervising RN.Documentation and Care Delivery
Communicates effectively and timely with physicians, patients, family members, and agency staff regarding patient care issues, visit schedules, and changes, coordinating care and delivering services as ordered by the physician and in compliance with laws and regulations.Documents patient visits thoroughly and accurately, maintaining current medication lists, and complies with policy and payer requirements, including timely documentation per LHC policy.Quality
Participates actively in performance improvement initiatives, aiming to reduce hospitalizations and improve patient outcomes and processes.Professionalism
Attends and participates in all meetings, including case conferences, punctually, with camera on for video meetings.Arrives to work with a positive attitude, willing to assist patients and the agency, and open to learning new skills.Teamwork
Flexibility in scheduling is required to provide care as needed, including participation in weekend rotations and observed holidays.Adheres to and participates in the agency's Episode Management process.Performs all other duties as assigned.Education and ExperienceLicense Requirements
- Current LPN licensure in the state of practice.
- Valid driver’s license, vehicle insurance, and access to reliable transportation.
- CPR certification required.
