Collaborates with physicians, nurses, other clinical disciplines, patient families and referral sources to coordinate patient care.

• Assesses patients and determines goals/priorities for counseling.

• Maintains accurate and complete records

• Possesses the skills and knowledge necessary to meet the age‑specific needs of the psychiatric and dual‑disordered patients including knowledge of growth and development as related to the course of the disorders and disease, the ability to assess and interpret data needed to identify each patient’s requirements relative to his/her specific needs

• Ensures timely and accurate communication of patient information with all members of the healthcare team.

• Maintains appropriate therapeutic boundaries with patients.

• Has knowledge of and provides substance abuse diagnosis‑specific care, and communication to patients and significant others related to their diagnosis.

• Assist and collaborates the patient’s discharge planning needs and readiness for discharge with other members of the healthcare team. Has working knowledge of local and state resources for discharge planning needs of patients.

• Demonstrates awareness and respect to the patient, family and/or significant other as an individual and family unit. (Identifies cultural and religious needs, language barriers, physical and psychological limitations, etc).

• Basic Cardiac Life Support must be obtained within 30 days of employment start date

• Bachelors Degree
