We are seeking a skilled and compassionate Family Nurse Practitioner to join our healthcare team. This role involves providing primary care services to primarily geriatric and adult patients. The Family Nurse Practitioner will collaborate with physicians, specialists, and interdisciplinary teams to deliver exceptional patient‑centered care in an outpatient setting.
Duties- Conduct thorough patient assessments including physical examinations, vital signs, and medical history documentation using EMR/EHR systems.
- Diagnose and manage acute and chronic illnesses across various specialties such as internal medicine and geriatrics.
- Document all patient interactions accurately within EMR/EHR systems like Athenahealth or eClinical
Works while maintaining compliance with NCQA standards. - Educate patients on family planning, nutrition, medication adherence, and disease prevention strategies.
