Hospice Patient Care Manager (RN) Lead hospice care without losing touch with the patients and families who make the work meaningful. In this hands-on leadership role, you will guide clinical operations across Corpus Christi and Kingsville, support field clinicians, uphold care quality, and step in for visits or caseload coverage when patient or staffing needs require it.
OpportunityThis role is ideal for an experienced hospice RN who wants Clinical Manager or Director of Nursing-level responsibility in a field-connected model. You will lead the clinical team, reinforce accountability, and help ensure patients receive coordinated, responsive hospice care wherever they are served.

Visits may take place in homes, skilled nursing facilities, long-term care, memory care, assisted living, and other care settings
Reliable transportation and a valid driver’s license are required
- Lead day-to-day hospice patient-care operations across both markets.
- Supervise, coach, and support clinical staff, including RN case managers, LVNs/LPNs, hospice aides, medical assistants, and interdisciplinary team members.
- Set clear expectations for visits, documentation, care-plan follow-through, productivity, and communication.
- Monitor clinical quality, compliance, patient/family concerns, and staff escalations.
- Partner with the interdisciplinary team on admissions, recertifications, IDG/IDT, plans of care, symptom management, medication coordination, and family education.
- Step into field coverage when needed, including in-home and facility-based hospice visits.
- Carry or cover a caseload when necessary; local guidance has referenced approximately 12-15 patients and about five visits per day during field coverage.
- Use the hospice EMR consistently and help ensure timely, accurate charting.
- Participate in shared weekday and weekend on-call coverage, including after-hours field response when clinically necessary.
Additional on-call and visit pay may apply.
What Makes You a Strong Fit- Active, unrestricted RN license valid for practice in Texas.
- At least one year of direct hospice nursing experience.
- In-home or field hospice experience is strongly preferred.
- Clinical leadership experience, whether through formal management or credible responsibility training, mentoring, coordinating, or holding clinical staff accountable.
- Comfort communicating with patients and families during decline, symptom escalation, and end-of-life situations.
- Willingness to travel across Corpus Christi and Kingsville and complete direct patient visits when needed.
- Willingness to participate in shared weekday and weekend on-call coverage.
- Experience as a Patient Care Manager, Hospice Clinical Manager, Director of Nursing, Director of Clinical Services, Assistant Director of Nursing, RN Team Lead, Hospice Supervisor, or Hospice RN Case Manager with leadership responsibility.
- Direct supervision of RN case managers.
- Homecare Homebase (HCHB) experience, or strong hospice/home-health EMR habits.
- Experience with hospice admissions, recertifications, IDG/IDT, plans of care, symptom management, medication coordination, family education, and after-hours triage.
