About This Role
The Senior LTSS Service Care Manager RN performs care management duties to assess and coordinate medical and supporting services for complex/high acuity populations. This role is ideal for experienced RNs looking to make a meaningful impact on patient care.
Highlights- Pay: $36.21 - $65.09 per hour
- Location: Corpus, US
- Schedule: Full Time, Day shift
- Evaluate the service needs of complex or high-risk members and recommend a plan for the best outcome.
- Develop and continuously assess ongoing long-term care plans and collaborate with the care management team.
- Coordinate and manage communication between members, families, and care providers.
- Monitor care plans and member status, revising as necessary to meet goals.
- Review member data to identify trends and improve quality care.
- Collaborate with healthcare providers to facilitate member services and treatments.
- Collect, document, and maintain member information to ensure compliance with guidelines.
- Provide education to long-term care members and their families on care options and benefits.
- Act as a liaison and advocate for members and families.
- Engage in a collaborative process with leadership to improve quality of care.
Education:
- Graduate from an Accredited School of Nursing or a Bachelor's degree required.
- Bachelor's degree in Nursing preferred.
- 4–6 years of related experience required.
Certifications:
- RN
- Registered Nurse
- State Licensure and/or Compact State Licensure required, OR - NP
- Nurse Practitioner
- Current State's Nurse Licensure required. - Resource Utilization Group (RUG) certification must be obtained within 90 days of hire required.
