The state supported living centers serve people with intellectual and developmental disabilities who are medically fragile or who have behavioral problems. The centers provide campus-based direct services and supports at 13 centers located throughout the state.

Key ResponsibilitiesThe QIDP II is central to the overall responsiveness and effectiveness of an individual’s active treatment program at an SSLC through oversight of the Individual Support Plan (ISP) process, which is centered on the individual’s preferences, strengths, and needs, and includes the creation of the ISP. The role includes coordinating, monitoring, documenting, and following up on all programs, services, and plans for individuals on the caseload.
Responsibilities include:
overseeing the interdisciplinary team (IDT), monitoring and providing oversight of active treatment programs, determining the need for program revision, resolving inconsistencies in training approaches, ensuring timely follow-up for IDT recommendations, facilitating skill acquisition and adaptive behaviors, promoting competent staff interactions, ensuring compliance with federal and state standards, and working with individuals who have complex needs and may exhibit aggressive or self‑injurious behaviors.
Additional duties: demonstrate strong organizational skills and balance competing priorities; work a flexible schedule as needed; exhibit strong computer skills within electronic records, database, and web‑based tracking systems; work independently with initiative and judgment.
Essential Job Functions (EJFs)Attend work regularly according to the shift schedule, accommodate rotation or overtime as required, and serve on call when needed. Orchestrate and oversee the ISP process, facilitate IDT meetings (ISP Preparation, Addendum, and Annual ISP), ensure assessments are conducted, coordinate supports and services, follow up on recommendations, observe individuals across settings, collect and analyze ISP data, modify plans based on progress, train and coach staff, collaborate with behavioral health staff, monitor and evaluate ISP implementation and documentation, advocate for individuals, and meet all documentation deadlines in the electronic records system.
