Job Responsibilities- Provide comprehensive outpatient primary care, including preventive services and chronic disease management
- Conduct patient evaluations, diagnoses, and individualized treatment planning
- Maintain accurate and timely clinical documentation
- Collaborate with multidisciplinary healthcare teams to optimize patient outcomes
- Participate in quality improvement initiatives and clinical leadership activities
- MD or DO degree
- Board Certified or Board Eligible in Family Medicine
- Active Oregon medical license or eligibility to obtain one
- DEA registration
- Current CPR certification
- Experience in outpatient practice or community health preferred
- Commitment to patient-centered, culturally responsive care
