Optum House Calls is seeking a Nurse Practitioner or Physician Assistant to provide annual in-home health assessments for Medicare Advantage and other health plan members. This field-based, non-prescribing role focuses on preventive care, health education, care gap closure, and collaboration with primary care providers to improve patient outcomes.

• Conduct comprehensive in-home health assessments, including medical history reviews, medication reconciliation, physical exams, and screenings
• Document diagnoses and findings to support care management and treatment planning
• Communicate assessment results to primary care providers
• Identify and address care gaps
• Educate patients on health conditions, medications, and treatment plans
• Assess social determinants of health and connect patients with appropriate resources
• Recognize and respond to urgent or emergent health concerns

• Nurse Practitioner:
  Active ANCC or AANP certification (Family, Adult, Geriatric, or Adult-Gerontology Primary Care)
• Active, unrestricted state license (or ability to obtain prior to start)
• Active driver’s license and reliable transportation
• Ability to travel within the assigned territory and perform in-home assessments

• At least 1 year of clinical experience, preferably in primary care, geriatrics, home health, or related settings
• Experience working with Medicare or geriatric populations
• Strong technology and EMR skills
• Ability to transport equipment and navigate varied home environments

• Independent, field-based practice
• Focus on preventive care and improving patient health outcomes
• No prescribing responsibilities
