• APP will support a busy Internal Medicine team seeing adult/geriatric patients.
• Nurse Practitioner will provide the co-management of chronic illnesses and conduct annual wellness visits as needed.
• The provider will join a busy primary care clinic and should be comfortable and confident caring for complex and co-morbid patients.
• Board Certified Family Nurse Practitioner, Adult Geriatric Nurse Practitioner, or Physician Assistant.
• 1+ years of experience working in primary care as an Advanced Practice Provider.
