JOB SUMMARY
Our Primary Care Practitioners provide high quality, comprehensive medical care and treatment to a diverse safety‑net patient population in a patient‑centered, team‑based model with an emphasis on prevention, health promotion, and chronic disease management. Practitioners manage panels of 700 to 1,200 patients and typically see about 14 patients per day. After‑hours calls are shared across the primary care group and supported by a nurse triage line.
Providers also have the option to teach medical residents and students.
Community Health Centers of Benton and Linn Counties is a Federally Qualified Health Center that opened in 2004. It serves more than 9,000 patients across five clinics in two counties, offering medical, behavioral, dental, and substance‑use–disorder care. The CHC partners with county public health and school‑based sites and operates under a high‑level Patient Center Primary Care Home certification.
RESPONSIBILITIES- Provide primary care services to a diverse patient population.
- Manage patient panels and coordinate comprehensive care with a multidisciplinary team.
- Participate in after‑hours call coverage as coordinated by the nurse triage line.
- Teach medical residents and students as part of the training program.
- Support program activities, including quality improvement and team‑based health promotion.
- Salary range of $75 to $85 per hour.

- Master’s of Science in Nursing from an accredited institution.
- Licensed as a Nurse Practitioner in Oregon or licensed in another state with the ability to obtain an Oregon license.
- Successful completion of a background check.
