Case Manager
StatusFull or part time hourly, exempt; may include some evenings, weekend work.
Organizational RelationshipsReports to Director of Operations

The Case Manager performs all the duties of the Social Service Assistant’s and provides Case Management services for men, women, and families enrolled in the Transitional Housing Program and/or in the ASSETS program. Case management services include screening/intakes, goal planning, appropriate referrals, regular meetings with clients, facilitation of life skills and other groups, and implementation of policies and procedures for record keeping.
Responsibilitiesand Duties
- Maintain professional attitude and behavior
- Abide by existing agency confidentiality policies.
- Perform all the duties of the Social Service Assistant
- Perform client intake interview to Transitional Housing Program
- Assist client with identification and appropriate implementation of objectives and goals while enrolled in the Transitional Housing Program.
- Conduct regular case management meetings with clients.
- Facilitate life skills or other group meetings.
- Maintain client charts according to agency guidelines
- Participate in multi-disciplinary staffing and present clients according to agency guidelines.
- Assure that client understands and adheres to program rules and expectations.
- Assist with special events and presentations.
- Provide ongoing reports and recommendations to the Director of Social Services.
- Gather and maintain program statistics.
- Collect, edit and process all required weekly, monthly and quarterly reports.
- Initiate new ideas for program improvement through the Director of Social Services
- Assist with on-going program development through recommendations to the Director of Social Services
- Perform other appropriate duties assigned by the Director of Social Services.
Bachelor's Degree in HDFS, Psychology, Social Work, or a related field. Must have knowledge of issues concerning the homeless population and have experience working with low-income and homeless individuals and families. Knowledge around issues of Domestic Violence, Mental Health, and Alcohol and Drug Abuse are highly desirable. Must have excellent abilities to collaborate and maintain positive relationship with other providers. Good organizational and communication skills are a must.
