Job Overview

We are seeking a few more dedicated, compassionate Licensed Practical Nurses (LPN) to join our brand new assisted living + memory care community in Coshocton!

Job :

Shifts Available:
Days (6:00 AM – 2:30 PM);
Afternoons (2:00 PM – 10:30 PM);
Nights (10:00 PM – 6:30 AM)

Hours Per Week: 24

Starting Rate: $27.50/hour

$1.00/hour shift differential between 2:00 PM – 10:00 PM

$0.50/hour shift differential between 10:00 PM – 6:00 AM

The ideal candidate will possess a strong background in long-term care, including experience with memory care and dementia.

• Deliver direct patient care by monitoring vital signs, administering medications, and performing routine assessments in accordance with physician orders and facility protocols.
• Assist residents with activities of daily living such as bathing, dressing, mobility, and nutritional support, ensuring dignity and comfort.
• Document all nursing interventions accurately within electronic health record (EHR) system, maintaining compliance with HIPAA regulations.
• Collaborate with interdisciplinary teams to develop and update individualized care plans tailored to residents’ needs, including those with Alzheimer’s disease or other memory impairments.
• Respond promptly to emergencies or changes in residents’ conditions by providing triage assessments and escalating concerns appropriately.
• Support infection control practices and maintain a clean, safe environment that meets all health standards for assisted living facilities.
• Educate residents and their families about health conditions, medication regimens, and wellness strategies to promote optimal health outcomes.

• Valid LPN license in good standing with relevant state licensing boards.
• Prior experience working in assisted living communities or long-term care settings; experience with dementia or memory care is highly desirable.
• Proficiency in electronic medical records (EMR) systems.
• Excellent communication skills for effective collaboration with residents, families, and team members.

From $27.50 per hour
