Hourly Salary Range: $53.00 - $58.00

• Complete initial, comprehensive, and ongoing assessments of patients and families to determine hospice needs.
• Provide a complete physical assessment and history of current and previous illnesses.
• Utilize all elements of the nursing process to provide professional nursing care.
• Write and initiate plans of care; regularly re‑evaluate patient and family/caregiver needs and revise plans as necessary.
• Use assessment data to determine nursing diagnoses and develop goal‑oriented care plans with patient and family participation.
• Record pain/SYMptom management and document changes or outcomes.
• Communicate with physicians regarding patient needs, report changes, and obtain or receive orders as required.
• Teach patients and caregivers self‑care techniques as appropriate.
• Work collaboratively with the interdisciplinary team.
• Perform all other duties as assigned.

• At least one (1) year of acute‑care experience in an institutional setting.
• One (1) year of experience in home care preferred.
• Current licensure in the state.
• Understanding of hospice philosophy and the needs of the terminally ill.
• Flexibility in work hours.
• Demonstrated nursing skills per competency checklist.
• Willingness to maintain comprehensive knowledge of information systems and applicable software programs.
• Ability to travel throughout the Bristol Hospice Service Area.
