Home Care Licensed Practical Nurse (LPN)
Enjoy independence, autonomy, and flexibility in the field to visit patients one on one in their home environment. The LPN will provide care coordination, disease and medication management, skilled wound care, catheter management, ostomy education, infection prevention, and home safety.

Full‑time;
Monday–Friday. Work includes a weekend and holiday rotation (approximately one weekend every 4‑6 weeks and two holidays per year). The rotation is dependent upon staffing levels.
Field work in the southeast metro area of Minnesota. The office is located in Roseville, MN.
Requirements- Current license as a Licensed Practical Nurse (LPN) in the State of MN. The license must be active and in good standing.
- 2 years experience as an LPN is preferred.
- Ability to travel within the geographic area served by the office. Valid driver’s license, reliable transportation, and proof of auto insurance are required.
- CPR Certification.
- Participate in ongoing interdisciplinary assessment of the patient.
- Provide services that are ordered by the physician as indicated in the plan of care.
- Recognize and report changes in the patient’s condition to the RN.
- Participate in the development and evaluation of the plan of care in partnership with the patient, representative (if any), and caregiver(s).
- Counsel and educate the patient and family in meeting healthcare related needs.
- Complete timely and accurate clinical notes including addressing patient’s progress.
