Home Health Licensed Practical Nurse

Center Well Home Health is seeking a compassionate and detail-oriented Home Health Licensed Practical Nurse to provide high-quality skilled nursing care to patients in their homes. As an LPN with Center Well, you will deliver personalized, evidence-based care that promotes patient autonomy, safety, and comfort while helping individuals manage acute and chronic conditions outside the traditional hospital setting. Working closely with an interdisciplinary team, you will follow individualized care plans developed by Registered Nurses and physicians, perform assessments within your scope of practice, administer medications and treatments, monitor vital signs and symptoms, and promptly report changes in patient condition.

You will educate patients and family caregivers on disease management, medications, wound care, and safety in the home, reinforcing teaching to support independence and reduce hospital readmissions. Documentation of all care will be completed accurately and promptly using electronic medical record systems to meet regulatory, payer, and Center Well quality standards.

In this role, you will travel to patient residences, assisted living facilities, or similar home-like settings to provide one-on-one care, building trusting relationships and adapting interventions to each patient’s environment and goals. You will coordinate with RNs, therapists, social workers, and other team members to ensure continuity of care, participate in case conferences, and contribute observations that help refine care plans.

Home Health LPNs at Center Well typically work a full-time schedule that may include some flexibility to accommodate patient needs, with mileage or travel between visits as part of the workday. You should be comfortable prioritizing a daily visit schedule, communicating effectively with patients, families, and providers, and using clinical judgment within your scope to elevate concerns promptly.
