Home Health RN (Registered Nurse)
Position SummaryWe are seeking an experienced and compassionate Home Health Registered Nurse (RN) to provide after-hours and weekend patient care within a 65-mile service area. This role is primarily responsible for on-call visits, symptom management, patient and family education, and delivering high-quality care in the home setting.
Schedule- After-hours coverage: 4:30 PM to 8:00 AM
- Weekend coverage:
Saturday and Sunday - Primary on-call role
- Up to 65 radial miles
- Provide after-hours home visits to patients as needed
- Assess and manage symptoms to ensure patient comfort and safety
- Perform medication management and education
- Deliver personal care and nursing interventions
- Support patients and families with education, guidance, and resources
- Educate families on disease progression and the dying process
- Complete admissions and required assessments
- Conduct symptom follow-up and PRN visits
- Perform recertification visits and documentation
- Collaborate with the care team to ensure continuity of care
- Active RN (Registered Nurse) license
- Prior hospice experience
- Strong clinical assessment and communication skills
- Ability to work independently in the field
- Experience with Homecare Homebase (HCHB) EMR system
- Excellent patient and family education skills
- Strong time management and organizational abilities
- Mileage reimbursement is provided between patient visits
- Mileage is not reimbursed from home to the first patient visit or from the last patient visit back home
