United Health Group in partnership with Northern Arizona Hospice seeks an experienced Registered Nurse Case Manager to deliver comprehensive hospice care under the Plan of Care and collaborate with the care team. You will evaluate needs, coordinate treatment, document progress, and support patients and families in challenging times.
Responsibilities include initial hospice evaluation within 48 hours, care plan initiation, ongoing reassessment every 15 days, and education of patient/family.
