Work Schedule:
Monday–Friday 7:30 AM–4:00 PM. Casual/PRN position. Work days/hours vary.
Accountable for the delivery of quality care to patients and families. The Nurse Navigator utilizes clinical expertise to guide patients, families and caregivers toward informed decision‑making and collaborates with a multidisciplinary team to provide timely cancer screening, diagnosis, treatment and supportive care.
Responsibilities- Provide appropriate, age‑specific care ensuring patient understanding and comfort.
- Perform initial and ongoing assessment to determine patient needs, incorporating data from the patient or significant others.
- Develop, review, and revise individualized plans of care, collaborating with providers and other disciplines, and providing patient‑specific teaching.
- Implement interventions consistent with the established plan of care following nursing protocols and current evidence.
- Perform technical nursing skills independently and safely, learning new equipment and techniques as required.
- Maintain a professional attitude, demeanor, and appearance; communicate effectively to solve problems and support interdisciplinary relationships.
- Initiate actions to prevent or minimize risks, complete appropriate reports, and communicate unresolved issues to relevant personnel.
- Act as financially responsible for human and material resources, accurately charging for supplies and equipment.
- Coordinate weekly clinic operations, schedule patients and physicians, collaborate with ancillary departments, update EMR, and order outside tests.
- Communicate treatment plans and follow‑up information with patients, families, and referring physicians in a timely manner.
- Guide patients and caregivers through informed decision‑making, education, and coordination of care across the cancer continuum, including enrolling patients in support programs.
- Promote a culture of safety by ensuring compliance with national patient safety goals, monitoring environmental safety issues, and reporting trends.
- Participate in Cancer Care Committee meetings and performance improvement activities, monitoring quality indicators and implementing improvements.
- Promote Cancer Services within the community through events such as health fairs and screenings.
- Graduate of an accredited nursing school (BSN preferred).
- Minimum 2 years of nursing experience.
- Previous ambulatory coordination experience.
- Current valid Registered Nurse (RN) license or equivalent; temporary permit allowed while awaiting licensure.
- American Heart Association or American Red Cross Basic Life Support (BLS) required within 3 months of hire.
- American Heart Association Advanced Cardiac Life Support (ACLS) required within 3 months of hire.
- Strong interpersonal and communication skills.
- Ability to multitask, prioritize assignments, and work independently.
- Proficiency in computer use for departmental and hospital information systems.
- Critical thinking and problem‑solving ability.
Medium work involving up to 50 pounds of force. Physical activities include climbing, crawling, kneeling, sitting, balancing, carrying, lifting, pulling, pushing, repetitive motions, stooping, twisting, and standing.
