This field‑based role enables associates to primarily operate in the field, traveling to client sites or designated locations as required, with occasional office attendance for meetings or training. The role requires residing within a reasonable commuting distance of the base location; otherwise, it will not be considered.
Responsibilities- Conduct face‑to‑face program assessments using predefined tools, applying motivational interviewing techniques to identify and evaluate individual waiver needs (LTSS/IDD, BH, PH).
- Use tools and predefined identification processes to spot members with clinical health care needs, coordinate their cases with interdisciplinary teams, and serve as the single point of contact.
- Manage non‑clinical needs of members with chronic illnesses, comorbidities, or disabilities to ensure cost‑effective use of long‑term services and supports.
- Document short‑ and long‑term service and support goals with the member’s chosen care team, including caregivers, family, natural supports, service providers, and physicians.
- Identify members who may benefit from alternative service levels or other waiver programs.
- Serve as mentor, subject matter expert, or preceptor for new staff, assisting with formal training and participating in process improvement initiatives.
- Submit utilization/authorization requests to utilization management with supporting documentation aligned to the care plan.
- Report critical incidents to appropriate internal and external parties (state and county agencies such as Adult Protective Services, Law Enforcement).
- Assist and participate in appeal or fair hearings, member grievances, appeals, and state audits.
- Travel to worksites and other locations as necessary.
- BA/BS degree and a minimum of 2 years of experience working with a social work agency; or an equivalent combination of education and experience.
- Specific education, years, and type of experience may be required based on state law and contract requirements.
- BA/BS degree in a health‑care related field preferred.
