Program SummaryHeartland Bridges houses two programs:
Bridges Transitional Living and Bridges Crisis Care. Transitional Living is a short‑term housing program enriched with supportive services for people with a diagnosed mental health or co‑occurring condition. Crisis Care provides a residential stabilization service for individuals needing a safe, secure environment for 3–5 days.
The Case Manager III (Overnight) provides facility oversight and supports the day‑to‑day operations of the program, responding to client needs. Decision‑making, problem solving, and interpersonal skills are essential.
Compensation & HoursSalary ranges from $19.74 to $23.54 per hour (based on experience) with four overnights per week (10:00 p.m.–8:00 a.m.).
Qualifications- Associate degree in a human services field (psychology, social work, mental health counseling, marriage & family therapy, or related field); bachelor’s preferred.
- Two years of experience in behavioral or mental health services.
- One year of experience working with adults with serious mental illness or substance use preferred.
- Valid driver’s license and acceptable driving record.
- Respond to crisis care referral calls, assess individuals, and provide accurate application verdict while maintaining confidentiality.
- Complete enrollment/intake for eligible Crisis Care clients.
- Develop supportive relationships with clients and respond to their needs during shifts.
- Document client activity per established record‑keeping procedures.
- Monitor self‑administration of medication and record it accurately; execute full medication count nightly.
- Conduct nightly housing lead searches and client room searches per protocol.
- Provide psycho‑education on life, social, and coping skills.
- Offer problem‑solving and crisis intervention to empower client self‑sufficiency.
- Apply trauma‑informed, person‑centered, and culturally competent principles in service delivery.
- Demonstrate knowledge of community and HFS resources to meet client needs.
- Review monthly client records.
- Orient clients to Bridges protocols and address non‑compliance to ensure safety.
- Attend scheduled staff meetings.
- Maintain facility upkeep.
- Work independently or as part of a team with little supervision.
- Display courteous and caring attitude toward clientele, volunteers, and visitors.
- Adhere to program and agency policies, procedures, and requirements.
- Collaborate with program staff, volunteers, and other agency staff.
- Participate in internal and external training opportunities.
- Create and maintain a dynamic self‑care plan.
- Perform other program duties as assigned.
