Position: SOAR Statewide Coordinator
Reports to: Support Services Manager
Compensation Status: Full-time, hourly
Hours: Approximately 37.5 hours per week
The SOAR Statewide Coordinator is responsible for overseeing and coordinating the implementation of the SSI/SSDI Outreach, Access, and Recovery (SOAR) program across the state. This position ensures that individuals who are homeless or at risk of homelessness receive high-quality assistance with applying for Social Security Disability Insurance (SSDI) or Supplemental Security Income (SSI) benefits. The role combines direct casework with systems‑level coordination, technical assistance, and collaboration with external partners to streamline and strengthen the SOAR process statewide.
ESSENTIAL JOB FUNCTIONS:Program Coordination and Oversight:
Coordinate statewide SOAR program activities and ensure consistent implementation of best practices across regions and partner agencies.
- Serve as the primary point of contact for SOAR-related initiatives
- Monitor statewide trends, challenges, and successes to inform program improvements.
- Build and maintain relationships with participants, providing emotional support and encouragement throughout the application process.
- Conduct comprehensive assessments of participant needs, including mental health and physical disabilities, to determine eligibility for SOAR services.
- Assist participants in gathering necessary documentation, such as medical records, to support their application.
Application Process:
- Guide participants through the preparation and submission of their SOAR application for SSI/SSDI benefits, ensuring all required documents are completed and submitted accurately.
- Collaborate with medical and social service providers to gather information and ensure participants meet the criteria for disability benefits.
- Ensure timely submission of all paperwork and advocate on behalf of participants to ensure swift processing of claims.
- Provide ongoing case management services, including monitoring progress, offering support, and connecting participants with other social services (e.g., housing, mental health treatment, substance abuse counseling).
- Maintain detailed case notes and track participant progress using appropriate data management systems.
- Work closely with internal and external stakeholders, including medical providers, social workers, and community-based organizations, to ensure participants receive comprehensive care.
- Participate in regular meetings and training sessions to stay up-to-date with SOAR processes, policies, and best practices.
- Serve as an advocate for participants during the appeals process if applications are denied.
- Provide crisis intervention and problem‑solving as needed, ensuring participants remain engaged in the process and do not fall through the cracks.
- Preferred Bachelor’s degree in Social Work, Psychology, or a related field (or equivalent experience).
- Experience working with individuals who are homeless, have mental health or physical disabilities, or have experienced trauma.
- Knowledge of the SSI/SSDI application process and the SOAR program.
- Strong organizational, communication, and interpersonal skills.
- Ability to work independently and as part of a team.
- Compassion, empathy, and a commitment to helping vulnerable populations.
Skills:
- Previous experience with case management, disability services, or social services.
- SOAR certification or experience with SOAR programs is a plus.
