CVS Health, through Signify Health, seeks a Full Time Clinician (NP/PA) to perform in-home health evaluations across designated coverage areas. Visits are about 45 minutes, including medical history, evaluation, and plan alignment with the health plan.
You will travel locally and within state licenses, using an iPad-based workflow, and provide diagnostic and preventive services as required. Active licenses and board certification are required, with multi-state licensure support.
