Signify Health is seeking a Full-time Clinician (NP or PA) to provide in-home health evaluations, visiting patients in their homes and bridging gaps in care. The visit lasts about 45 minutes and includes history review, physical exam, and optional tests per health plan orders.
You will join a national network of clinicians, travel locally as needed, and deliver personalized health evaluations in members' homes while using an iPad workflow.
