Position: Registered Nurse Care Manager / Case Management (RN)

Registered Nurse Care Manager / Case Management (RN) at Yale New Haven Health summary:
The Registered Nurse Care Manager coordinates individualized nursing care plans, assesses patient conditions continuously, and ensures adherence to hospital policies and procedures. This role involves acting as a clinical resource, participating in staff education, and maintaining compassionate patient‑centered care. Preferred experience includes geriatric psychiatric nursing and previous hospital work, requiring relevant licensure and training certifications.

Responsibilities
  • Assesses the patient's physical and psychological condition on an ongoing basis throughout the continuum of care. Documents appropriately the information obtained from the ongoing assessment and shares significant assessment data with the appropriate discipline.
  • Adheres to The Westerly Hospital policies and procedures.
  • Acts as a clinical resource and role model.
  • Actively participates in training and educational activities for other staff members.
  • Demonstrates a high level of respect, dignity, compassion, and professionalism for all customers by utilizing the four tenets of Patient Family and Centered Partnership.
  • Recognizes and values the contributions of fellow employees and displays cooperation and teamwork.
  • Reports to work on time, prepared to work, and follows hospital policies for notice of lateness/absenteeism and time off requests.

Qualifications Education

Graduate of an accredited school of nursing required.

Experience

Previous Geriatric Psychiatric experience preferred; previous hospital experience preferred.

Licensure

Current Rhode Island license, BLS required. CPI training within 1 year of employment.

Special Skills

Previous geriatric psychiatric experience preferred.
