Pride Health is seeking an RN Case Manager to travel between two Skilled Nursing Facilities in New London County, CT. The schedule is Monday to Friday, 8am–5pm, on a 3-4 month contract with a pay rate of $43-46/hour.
The role requires an active RN license, BLS certification, and at least 1 year of clinical experience in a long-term care setting. The provider team will rely on your expertise in geriatrics and chronic illness to coordinate care and documentation.
