Family Medicine Physician or Advanced Practice Provider (Nurse Practitioner or Physician Assistant)

We are seeking a dedicated and compassionate Family Medicine Physician or Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our outpatient clinic team. This role offers an excellent opportunity to deliver comprehensive, patient-centered primary care in a supportive and collaborative environment. The successful candidate will play a vital role in promoting health and well-being for patients of all ages through high-quality, evidence-based care.

Key Responsibilities:
  • Provide comprehensive primary care services to patients of all ages, including preventive screenings, health assessments, and wellness counseling
  • Manage acute illnesses and chronic conditions through evidence-based treatment plans
  • Educate patients on health promotion, disease prevention, and lifestyle modifications
  • Collaborate with a multidisciplinary care team to coordinate patient care and ensure seamless service delivery
  • Document patient encounters accurately and maintain up-to-date medical records
  • Participate in quality improvement initiatives and clinical team meetings
Skills and

Qualifications:
  • Medical degree (MD or DO) for physicians, or valid certification/license for Nurse Practitioners and Physician Assistants
  • Current state medical license and board certification or eligibility in family medicine or primary care
  • Proven experience in outpatient primary care or family medicine preferred
  • Strong communication and interpersonal skills to build trusting patient relationships
  • Ability to work effectively within a team and adapt to a fast-paced clinical environment
  • Commitment to providing compassionate, patient-centered care and continuous learning
