Position SummaryThe Support Coordinator assists individuals with intellectual and developmental disabilities in accessing, coordinating, and monitoring community-based services. This role focuses on care coordination, service planning, and ongoing support to help individuals achieve personal goals and maintain quality outcomes.
Key Responsibilities- Coordinate and manage individualized service plans (ISP)
- Conduct assessments and gather clinical/social information
- Facilitate ISP meetings and ensure annual updates
- Connect individuals to community resources and services
- Assist with Medicaid eligibility and recertification
- Maintain monthly face‑to‑face contact per guidelines
- Monitor service delivery and outcomes
- Document services and maintain compliance with DBHDD and HIPAA
- Respond to participant needs, concerns, and crises
- Collaborate with families, providers, and interdisciplinary teams
- Bachelor’s degree in human services field (required)
- 1+ year experience in case management or developmental disabilities services
- Master’s degree may substitute for experience
- Valid driver’s license and ability to travel locally
- Must reside within assigned coverage area (preferred)
- Care coordination and case management
- Strong communication and relationship-building
- Problem‑solving and critical thinking
- Knowledge of Medicaid/waiver programs
- Microsoft Office and documentation skills
This is a mobile, field‑based position requiring travel throughout the assigned counties. Work includes home visits, community settings, and occasional remote/administrative tasks. Flexibility for evenings or weekends may be required.
