Patient Navigator – Healogics, LLC
Healogics is the largest provider of advanced wound care services in the United States, treating more than 300,000 chronic wound patients annually across over 600 sites. With an aging society, rising obesity and diabetes, and an uptick in surgical procedures, the number of patients with non‑healing wounds that would benefit from expert care is dramatically increasing. The company is working to provide differentiated, quality outcomes to as many patients as possible through its outpatient clinic partnerships.
Responsibilities- Coordinate with center leadership to maximize daily patient census.
- Actively participate in staff meetings to support key functions within the center.
- Greet patients and other visitors, answer and route calls to appropriate staff.
- Maintain all patient communication needs including scheduling, rescheduling, and appointment reminders; also work and schedule for provider according to the care continuum model.
- Obtain and verify patient insurance information, including pre‑certifications and pre‑authorizations for services, and enter data in appropriate databases.
- Collect and enter patient charges in databases.
- Verify and reconcile charges as directed; prepare monthly patient survey data and appropriate documentation, then transmit to providers.
- Assemble new patient charts, maintain and file existing patient charts, and spot‑check charts for data completeness and signatures.
- Coordinate/schedule ancillary testing with other hospital departments.
- Arrange for patient transportation as needed.
- Maintain office equipment and supplies as needed, and medical supplies as directed.
- Perform other duties as required.
Education and Experience
- High School Diploma or General Education Development (GED);
Associate’s degree preferred. - Minimum of two (2) or more years of office administration experience, preferably in a medical setting; prior medical coding experience preferred.
Skills and Abilities
- Proficient in Microsoft Office (Word, Excel, Outlook).
- Good customer, interpersonal and communication skills, both orally and in writing.
- Organization and time‑management skills.
- Ability to type 60 words per minute (wpm).
- Basic math skills.
- Attention to detail.
- Ability to maintain confidentiality.
- Ability to work in a fast‑paced environment and on multiple projects simultaneously.
- Ability to work with others in a team environment.
- Being in a stationary position for extended periods of time (4 hours or more).
- Viewing computer screen for extended periods of time (4 hours or more).
- Keying frequently on a computer for 4 hours or more.
- Reading, communicating, detecting sounds by ear, close, distance and peripheral vision.
- Lifting/moving items up to 75 pounds with equipment assistance.
- Repetitive motions, bending/stooping, writing.
- Patient care environment.
The hourly rate for this position generally ranges between $17.81 and $21.90 per hour, depending on qualifications, experience and location.
