- BS or BA in social work or in a related behavioral science field such as psychology sociology, education, family studies, etc., with a minimum of one year full time experience working with individuals in a human service setting after completing the BA or BS or be a certified alcohol and drug counselor. A Master’s degree in social work or related behavioral health field may be substituted for the one year of experience.
- Must successfully complete case management training within 6 months of hire and annual continuing education requirements.
- Organizational skills and basic computer skills.
- Must have a current driver’s license and driver’s insurance.
- Demonstrates ability to work as an effective member of a multi-disciplined team.
- Demonstrate proven sensitivity to the various cultural and socioeconomic characteristics of OQH clients and staff.
The Targeted Case Manager is responsible to assist clients and families in accessing behavioral health treatment, medical interventions, educational, social, development, and other support services. Targeted Case Manager advocates for the needs of the clients and families and works closely with our Outpatient clinical team and community partners to ensure the client’s needs are being met across settings. Targeted Case Managers focus on linking clients to local resources and enhancing social and emotional functioning and life skills.
EssentialJob Duties
ADMINISTRATIVE: 25%
- Any youth who is considered ineligible for services is referred back to the referral source with recommendations as to a more appropriate course of action or service.
- To follow the safety guidelines and practices as identified in the program operations manual when providing services and provide a daily itinerary to their supervisor or other assigned person, noting where they will be located at all times throughout the work day.
- Case Manager will notify parent/legal guardian or other referral agencies of all critical incidents within 24 hours.
- Attend supervision by a behavioral health professional who has completed case management training at least twice per month with at least one of these supervisory contacts being on an individual basis and face-to-face.
- Ensure client’s needs are fulfilled and follow administrative protocols for request such as vouchers, use of credit cards, training request, etc.
- Attend case consultation and clinical meetings.
DOCUMENTATION: 25%
- If contact with the client and family does not occur within the required time frames, the case manager is responsible to document the reason in the case record and will continue to make effort to contact the family via telephone and letter, all of which will be documented in the record.
- Development of a specific care plan with the client with periodic revisions with specify the goals and actions to address the medical, social, educational, or other services needed.
- Documentation must adhere to contract requirements following state regulations for case records, case notes, and contain the achievement and progress on goals.
- Discharge plans must be completed on all clients who have had three or more service contacts.
- Documentation must follow time frames as stated in the program operations manual.
DIRECT SERVICES: 45%
- Complete additional releases of information forms with the client, family, and/or legal guardian, in order to have the ability to gather information needed in developing the care plan.
