Signify Health, a CVS Health company, is hiring a Full-Time Clinician (NP or PA) to perform in-home health evaluations nationwide. You will visit patients in their homes (40 hours/week), review meds and history, perform physicals, and connect patients to appropriate care.
You’ll use an iPad with a clinical workflow, may travel locally, and conduct virtual visits as needed. Requires board certification and unrestricted licensure; multi-state licensure preferred.
