PRN Occupational Therapist (OT) Aventura at Walton Hills – Walton Hills, OH

Aventura at Walton Hills is seeking a compassionate and motivated PRN Occupational Therapist (OT) to join our in-house rehabilitation team. This is an excellent opportunity for a therapist looking for flexible scheduling while making a meaningful impact on the lives of residents in a skilled nursing and rehabilitation setting.

• Evaluate residents and develop individualized treatment plans.
• Provide skilled occupational therapy services to short‑term rehabilitation and long‑term care residents.
• Collaborate with an interdisciplinary team to maximize patient outcomes.
• Complete timely and accurate documentation using Net Health.
• Maintain compliance with state, federal, and company regulations.

• Current Occupational Therapist license in the State of Ohio.
• Skilled nursing or long‑term care experience preferred, but not required.
• Experience with Net Health and Point Click Care  (PCC) is preferred.
• Strong clinical, communication, and documentation skills.
• Ability to work independently while collaborating effectively with the rehab team.
