Signature HealthCARE is seeking a Social Services Director to identify and address residents’ social, emotional, and psychological needs and support discharge planning. You will supervise documentation and participate in care planning while serving as a resource for residents and families.
A high standard of confidentiality and professionalism is required. Required are a Bachelor’s or Master’s in Social Work or Psychology and 3+ years in social services, preferably in healthcare.
