We are seeking a PRN Physical Therapist (PT) to join our dedicated rehabilitation team. This position offers flexibility and the opportunity to make a meaningful impact on residents' lives in a rewarding setting.

Position Summary:
As a PRN Physical Therapist, you will evaluate and treat residents to improve their mobility, strength, and functional independence. You will work collaboratively with a multidisciplinary team to deliver high-quality, individualized care.

Key Responsibilities:

  • Perform comprehensive evaluations of residents' physical abilities and needs.
  • Develop and implement personalized treatment plans to improve strength, mobility, balance, and overall function.
  • Provide therapeutic interventions, including exercises, manual therapy, and training on assistive devices.
  • Monitor residents' progress and modify therapy plans as needed to achieve goals.
  • Collaborate with an interdisciplinary team to ensure holistic care.
  • Educate residents and their families on therapy goals, exercises, and strategies for continued improvement.
  • Accurately document evaluations, treatment sessions, progress notes, and discharge plans in compliance with regulatory and facility standards.
  • Participate in quality improvement initiatives and contribute to the rehabilitation department's success.
Qualifications:
  • Doctorate or Bachelor's/Master's degree in Physical Therapy from an accredited program.
  • Current and valid Physical Therapist license in the state of Colorado.
  • Experience in skilled nursing or rehabilitation settings is preferred.
  • Strong communication and teamwork skills with a resident-centered approach.
  • Ability to work independently and adapt to a flexible PRN schedule.
  • Dedication to providing compassionate and effective therapy services.
