Masonic Home of Louisville is seeking a Social Worker to provide psychosocial support, assessment, and discharge planning for residents on the rehabilitation unit. You will manage a caseload and serve as the primary contact for residents and families throughout short- or long-term stays.
You will coordinate discharge planning, collaborate with the interdisciplinary team, arrange home health services, and ensure safe, timely transitions of care while documenting progress in the EMR.
