Avid Health at Home is seeking a PRN Physical Therapist Assistant (PTA) to provide skilled home health services under a licensed Physical Therapist's supervision in the patient's home.
Responsibilities include implementing therapy plans, monitoring progress, educating patients and caregivers, and documenting care in accordance with Medicare guidelines and agency policies. Travel to patients' homes within the service area is required; strong communication with the supervising PT is essential.
