Oral Health Community Health Worker

• Provide outreach and follow-up services for potential clients and individuals receiving public health dental hygiene services
• Visit homes and other sites to promote services, schedule appointments, and conduct necessary follow-up
• Work with dental hygiene teams in schools, clinics, long-term care facilities, child care settings, Head Starts, and other community settings
• Document services in appropriate electronic or paper health records
• Support the oral health program as assigned
• Participate in medical services clinics and administer screening tests and measures
• Follow up on referrals for oral health services
• Conduct medical, social, or environmental assessments in homes
• Participate in developing training and train community health workers and volunteers
• Organize community groups promoting health services and participate in community group discussions
• Record activities, services, test results, and prepare documents for data tabulation, analysis, and patient referral

• High School diploma or GED
• One (1) year of experience in public health or in another position with the provision of social services, supportive services, personal care, or related duties, or an equivalent combination of experience and education
• Possess a valid motor vehicle operator’s license
• Community Health Worker Certification
• May be required to submit to a drug screening test and background check according to the agency’s policies
• Will be required to become certified as a Community Health Worker within 6 months if not already certified

Demonstrates expertise in community outreach, client follow-up, and health service promotion, with a strong focus on documentation and assessment in public health settings. Capable of training community health workers and organizing community health initiatives.
