North Key Community Care is seeking an Assertive Community Treatment Care Coordinator.
The ACT Care Coordinator serves as a member of the multidisciplinary Community Integration Team, providing community-based care coordination, outreach, rehabilitation support, case management, and recovery-oriented services to individuals with serious mental illness and co-occurring disorders. Services are delivered through Assertive Community Treatment (ACT) to promote recovery, stability, independence, and successful integration within the community.

Varied as Assigned (Community-Based Services, Client Residences, Hospitals, Courts, Correctional Facilities, Personal Care Homes, and Other Community Locations)

QUALIFICATIONS
  • A bachelor's degree from an accredited college or university is required. Degrees in Social Work, Psychology, Human Services, Sociology, Behavioral Science, or other related disciplines are strongly preferred.
  • At least one year of experience in behavioral health, substance use treatment, case management, rehabilitation services, or community support services is preferred.
  • Experience working with ACT, AOT, forensic, or community behavioral health populations preferred.
  • Valid driver's license, insurance, acceptable driving record, and reliable transportation required.
JOB DUTIES Care Coordination and Community Support Services
  • Engage clients in a structured, collaborative relationship that promotes recovery, self-sufficiency, independence, and community integration.
  • Provide community-based services in homes, hospitals, courts, correctional facilities, shelters, and other community settings.
  • Participate as an active member of the multidisciplinary ACT treatment team.
  • Coordinate medical, behavioral health, housing, employment, educational, transportation, and social support services.
  • Assist clients in obtaining and maintaining benefits, insurance coverage, and community resources.
  • Provide outreach services to individuals who are difficult to engage in traditional clinic-based treatment.
  • Maintain frequent face-to-face contact with clients in community settings.
  • Support clients in developing daily living, independent living, social, vocational, and community functioning skills. Coordinate and support services for individuals with serious mental illness, co-occurring disorders, substance use disorders, or developmental disabilities.
  • Identify and address risks and gaps in care that may contribute to hospitalization, emergency room utilization, housing instability, or reduced functioning.
  • Provide education to clients, families, and natural supports regarding recovery, wellness, and available community resources.
  • Assist with crisis response, safety planning, and coordination of appropriate services in collaboration with the treatment team.
  • Provide transportation to clients, as clinically appropriate and in accordance with North Key policies, to facilitate access to medical and behavioral health appointments, court hearings, community resources, treatment services, and other necessary activities.
  • Provide emergency after-hours on-call coverage during scheduled after-hours shifts. Responsible for answering and responding to after-hours phone calls regarding program and client concerns and providing an onsite response when indicated.
  • Coordinate care during transitions between hospitals, correctional facilities, residential settings, and community-based services.
Documentation and Quality Assurance
  • Complete all required documentation accurately and within established timelines.
  • Complete reports, referrals, prior authorizations, and other required records promptly.
  • Monitor and document progress toward treatment plan goals.
  • Monitor service utilization and assist in maintaining reimbursable services.
Professional Responsibilities
  • Complete all mandatory training and professional development requirements.
  • Maintain HIPAA compliance and client confidentiality.
