Apex Home Health, Hospice & Home Care seeks a Full-time Clinician (Nurse Practitioner or Physician Assistant) to perform In-Home Health Evaluations across a defined coverage area. You will engage patients in ~45-minute visits, review medications and medical history, and perform physical exams with ordered diagnostic tests.
Travel is extensive, up to 100%, including overnight stays. Must hold active, unrestricted licenses, be board-certified, and qualify for multi-state licensure as needed.
