Medical Director – Skilled Nursing Facilities / Long-Term Care
Position SummaryThe Medical Director provides clinical leadership and medical oversight for nursing home and long-term care facilities. This role ensures high-quality, patient-centered care for residents while collaborating closely with nurse practitioners, nursing leadership, attending physicians, and interdisciplinary teams. The Medical Director supports regulatory compliance, quality improvement initiatives, and development of clinical best practices across facilities.
Key Responsibilities- Provide medical direction and oversight for resident care within skilled nursing and long-term care facilities.
- Establish and maintain clinical standards, protocols, and best practices.
- Support management of complex geriatric and chronic medical conditions.
- Participate in development of care plans and clinical pathways.
- Review and address quality metrics, hospitalization rates, infection control practices, and patient outcomes.
- Collaborate closely with nurse practitioners in the delivery of resident care.
- Provide mentorship, supervision, and clinical guidance to advanced practice providers.
- Conduct regular case reviews and interdisciplinary discussions.
- Support timely evaluation and management of acute clinical changes in residents.
- Promote team-based, evidence-based care delivery.
- Ensure compliance with federal, state, and local regulations governing nursing homes and long-term care facilities.
- Participate in surveys, audits, and quality assurance activities.
- Assist with policy development and implementation related to clinical care and safety standards.
- Support infection prevention and emergency preparedness initiatives.
- Serve as a clinical leader and liaison among administration, nursing staff, providers, residents, and families.
- Participate in Quality Assurance and Performance Improvement (QAPI) programs.
- Lead initiatives to improve patient satisfaction, reduce avoidable hospitalizations, and optimize clinical outcomes.
- Promote a culture of accountability, professionalism, and compassionate care.
- Promote care delivery at the right time, right place.
- Attend staff and interdisciplinary meetings as required.
- Maintain accurate and timely documentation.
- Assist with credentialing and clinical review processes.
- Be available for consultation regarding resident care concerns.
- MD or DO degree from an accredited institution.
- Board certification in Internal Medicine, Family Medicine, Geriatrics, or related specialty preferred.
- Active and unrestricted medical license in the state of practice.
- Experience in skilled nursing facilities, long-term care, post-acute care, or geriatrics preferred.
- Experience of collaborating with nurse practitioners and interdisciplinary teams.
- Strong leadership, communication, and organizational skills.
- Prior Medical Director experience in nursing homes or post-acute care settings.
- Familiarity with CMS regulations and long-term care compliance standards.
- Combination of onsite facility rounding and administrative responsibilities.
