Position: Nurse Practitioner-32 hours/week

Position Summary

The Nurse Practitioner will facilitate HWR’s efforts to relieve suffering, enhance comfort, promote quality of life and foster choice in the presence of an advanced serious illness for all patients and families. This role focuses on clinical practice, education, collaboration, consultation and research related to palliative care. The scheduled shift structure is 32 hours per week, divided into four 8‑hour shifts.

Clinical

Practice
  • Participates in development and implementation of new programs that affect clinical practice.
  • Performs holistic patient assessment and prescribes symptom management or recommends management to primary care providers.
  • Conducts medical history and physical exam in collaboration with physicians to develop a plan of care.
  • Coordinates care with team members and healthcare professionals involved in the patient and family plan of care.
  • Reviews medical records, including physician/consultant notes, lab and diagnostic test results, and formulates a plan of care based on the data.
  • Provides follow‑up to reassess the plan of care as determined by patient’s goals.
  • Participates in analyzing and evaluating clinical practice standards in terms of cost and quality.
  • Provides independent advanced practice nursing care within the scope of practice as delineated in the State of Ohio.
  • Participates in development and implementation of new standards of practice for hospice and palliative care.
Consultation
  • Ensures quality care across the continuum as a palliative care expert.
  • Facilitates and/or participates in transdisciplinary team conferences.
  • Serves as a consultant to staff caring for patients in hospitals, long‑term care settings and home settings.
  • Functions as an expert consultant to healthcare professionals at local, national and international levels to influence and advance clinical practice in palliative care.
Education
  • Maintains thorough knowledge of current practice and literature relating to palliative care and facilitates sharing with staff.
  • Educates patients and families on applicable areas of palliative care, such as interventions for pain and symptom management and choice in end‑of‑life care.
  • Empowers patients and families to identify and meet goals of care.
  • Provides education to HWR staff in areas of palliative care.
  • Provides education to outside agencies, hospitals and long‑term care settings on principles of palliative care.
Collaboration
  • Maintains a standard care agreement with HWR Medical Directors.
  • Works as part of a team.
  • Collaborates with members of the palliative care team.
  • Collaborates with healthcare professionals as appropriate within the community.
  • Facilitates communication between nursing staff/physicians and patient/family.
  • Participates on HWR committees.
  • Participates in quality improvement activities.
  • Maintains and cultivates professional relationships in healthcare that strengthen HWR’s strategic marketing plan.
  • Fulfills professional growth through involvement in professional organizations, presentations, and publications.
Research
  • Participates in agency research endeavors.
  • Present findings of research internally and at professional conferences.
Accountability
  • Participates in peer reviews.
  • Adjusts to changes in workload and schedules based on changing departmental and organizational priorities.
  • Maintains national certification.
  • Seeks palliative care educational opportunities to increase knowledge.
  • Teaches and lectures for continuing education programs.
  • Present at multidisciplinary patient care conferences.
  • Serves as a preceptor to Advanced Practice Nurses.
Success Capabilities
  • Maintains a current Registered Nurse license in the State of Ohio.
  • Maintains a current Certificate of Authority in the State of Ohio.
  • Master of Nursing degree.
  • Minimum of two years in a clinical setting.
  • Must be flexible and have problem‑solving skills, prioritize, and demonstrate healthy coping strategies.
  • Ability to demonstrate excellence in written and oral communication.
  • Ability to meet patient/family needs that require variable hours.
  • Knowledge of hospice and palliative care clinical standards of practice, policies, and procedures.
