Job Description Overview
Nurse Practitioner State Traveler - Full Time State Traveler, Home Risk Assessments, Assignments: 4 weeks on then 1 week off.
About the PositionAs a Nurse Practitioner State Traveler, you will deliver primary care from patients' homes across the state, conducting up to one‑hour visits to assess health status, medical history, and social environment, coordinating care and bridging gaps.
Responsibilities- Perform detailed Adult/Geriatric Assessments, diagnosing, treating, and educating patients.
- Coordinate care plans and ensure meticulous follow‑up.
- Collaborate with medical professionals and families.
- Engage in home visits and digital consultations.
- Master’s degree and RN/NP licensure.
- A minimum of three years as a Nurse Practitioner.
- Board certification by AANP, ANCC, or AACCN in an approved specialty.
- Current RN/NP licensure in the state of practice with prescription authority or ability to obtain it.
- Current BLS, ACLS, or CPR certification.
- Strong verbal and written communication skills.
- Comfortable and flexible with frequent change.
- Travel up to four weeks for assignments, holding a valid driver’s license and meeting automobile insurance standards.
Travel to homes, skilled nursing facilities, or assisted living facilities within the state.
Workload- Monday – Friday, Saturdays optional.
- Full‑time work week expected.
- Four weeks on followed by one week off.
$110,000 - $120,000 per year based on geographic location and experience. Enticing bonus opportunities.
