Job Description
RN Case Manager – Full Time, Flexible Schedule. Monday–Friday 8‑4, On‑call rotation with bonus pay.
Location Covered:
South Franklin County and Madison County. Mileage reimbursement.

Position SummaryWe are currently seeking a dedicated and compassionate Registered Nurse Case Manager to join our Home Health team. The ideal candidate will embrace our mission and provide skilled nursing care while coordinating the overall care plan for hospice patients and their families.
Responsibilities- Coordinate the overall care plan for hospice patients.
- Provide skilled nursing care to hospice patients and support to families.
- Collaborate with an interdisciplinary team to deliver comprehensive care.
- Education:
Graduate of an accredited nursing program with a current RN license. - Experience:
Minimum of two years of RN experience required; previous hospice or home health experience strongly preferred.
